The patient portal is a secure web portal that allows you to communicate with our office via secure messaging.  As a patient you will have access to your medical records and be able to view labs, submit pharmacy refill requests, pay bills and schedule appointments.  

Policies and Limitations:

  • All users must be established patients of Grand Junction Gastroenterology. 
  • The portal is not to be used for emergencies.  If there is an emergency dial 911 or go to the nearest emergency room.
  • No triage or treatment requests.  Diagnosis and treatment can only be provided after the patient has seen the provider. 
  • Narcotic refills or controlled substance requests may not be made through the patient portal. 
  • Medication refills only for medications prescribed by our providers.
  • All email communication will be done through the patient portal.  Communication is not allowed via direct email and must only occur through secure messaging.  Standard email lacks security and privacy and may expose confidential information to unintended parties. 
  • Allow 72 hours for communication back through the portal.  If you do not receive a response in 72 hours please contact our office.
  • We strive to keep your records accurate and complete.  If you identify any discrepancy in your records please notify our office.
  • By signing up for the patient portal you allow Grand Junction Gastroenterology to send your health information directly to you through our secure messaging system. 

Protecting your Private Health Information and Risks:

We strive to protect your PHI and ensure that all communication is kept secure through the portal.  However keeping it secure depends upon two additional factors:  the secure message must reach the correct email address, and only the correct individual (or someone authorized by that individual) must be able to gain access to it. 

  • Confirm your name and personal information is correct.  Provide us with your correct email address and notify us if it ever changes. 
  • Do not store messages on your employer-provided computer.  You should never use a public computer to gain access to your health information. 
  •                                       
  • Protect your password.  Use screen savers and close messages after you have read them.  If you think someone has learned your password change it.  If you need to change your password use the “forgot password” link on the login page.    
  • Our office is not responsible for breaches of confidentiality caused by you or an independent third party. 
  • You are responsible for taking steps to protect yourself from unauthorized use of online communications. 
  • Follow-up is your responsibility.  You are responsible for scheduling any necessary appointments and for determining if an unanswered online communication wasn’t received. 
  • Our office is responsible for protecting your health information.  We are not liable for improper disclosure of confidential information unless it was caused by our office. 
  • All messages sent to you will be encrypted. 
  • Staff members other than your provider will be involved in receiving your messages, and routing them to the provider, nurse, or front desk as necessary.

I acknowledge that I have read and fully understand this consent form.  I have been advised of the risks and benefits of the patient portal and agree that I understand the risks associated with online communications between my provider and myself, and consent to the conditions outlined herein.  I acknowledge that using the patient portal is entirely voluntary and will not impact the quality of care I receive from Grand Junction Gastroenterology should I decide against using the patient portal.  In addition, I agree to adhere to the policies set forth herein, as well as any other instructions or guidelines that my physician may impose for online communications.  I have been proactive in asking questions related to this consent agreement.  All of my questions have been answered with clarity.
